Brighten the day providers' area
Welcome to the providers' area for our holiday and food activity programme, Brighten the Day.
Once your holiday club has finished, please complete the online forms. Please submit a separate form for each holiday club you manage.
There are two forms, a return form which asks for the numbers of children who attended and a monitoring form which asks for more details about the children and also asks for your feedback on the current programme.
The information you supply will help us produce the report that we need to give to the Department of Education, this is a key part of our funding. Your feedback will also help us develop future Brighten the Day programmes.
The deadline to complete these forms is Wednesday 24th April 2024

To complete this form you will need to give details of how you managed your bookings and how you spent your funding.
To complete this form you will need to provide numbers of children who attended the sessions and whether they attend primary or secondary school. You will also need to give the number of children who receive free school meals and who are SEND.
