In this role, you will play a key part in the delivering Activity Based Care Programme, which provides person-centred care following individual enablement plans, relating to Adults with Learning Disabilities, with a wide range of complex care and behaviour support needs. You will be delivering a variety of community and building based activities, geared towards the individual's needs and aspirations -offering a high level of support to each service user, in accordance with their personal care and support plan. You will be able to support the individual to achieve agreed enablement goals, gain new or improved skills promote confidence and community engagement alongside encouraging new experiences.

This carer shares their home and community life to support the person to live their highest quality of life. Arrangements can be on a long-term or short-term basis.
