In the last two years, have you accessed free contraception, sexual health screening or treatment for a sexually transmitted infection?

We want to hear about your experience of using sexual health services in Gateshead, whether that's through a pharmacy, GP practice, sexual health clinic or online.

Your responses are completely confidential.

At the end of the survey, you can choose to enter a free prize draw for the chance to win a Love2Shop voucher worth £100 (any personal information you provide for the draw will be handled separately from the survey data so that your survey response remains completely anonymous).
